Diabetes is a chronic disease characterized by increased glucose level in the blood. It may affect any person irrespective of gender, ethnicity or age. The complications related to diabetes include nephropathy, neuropathy, retinopathy etc. Diabetes is a major cause of morbidity and mortality in US and causes a significant burden to the health care system. Hence, it requires proper management in order to reduce the cost incurred on healthcare systems and improve the quality of care and life of the patient (Zhou et al., 2020)
In this assessment, the implementation of intervention plan for the patient diagnosed with diabetes will be discussed in detail. I spent 3 practicum hours with Mr. Mathew, a 45 years old male Caucasian patient diagnosed with diabetes for the past 20 years. Special areas of discussion include leadership and change management, nursing ethics, tools applied to educate patient on the intervention plan, communication and collaboration, nursing board and state policies and the impact of intervention in quality of care, safety and cost.

Intervention for the management of diabetes should be done in a comprehensive manner involving the patient, family, health care team and relevant state or federal agencies.
